Primary Care When and Where You Need It

If you have difficulty leaving your home, Care Dimensions' HomeMD services can come to you wherever you are, whether you live in a private residence, rest home, or an assisted living facility.   

Our services mirror those you would find in a traditional primary care office-based clinic. In addition to routine care and disease management visits, our providers can treat you for acute illness or follow up with you after being discharged from the hospital or skilled nursing facility. You can even receive your vaccinations and flu shots through HomeMD, and we can arrange for in-home laboratory and x-ray services.

Who Qualifies for HomeMD?

Because we work with your insurance provider, our services are often available at no additional cost to you.

To qualify for HomeMD care you must:
  • Have difficulty leaving home
  • Live in a private home, assisted living community, or rest home.
  • Be covered by Medicare, Medicaid, most commercial plans, or supplemental/Medigap insurance  
  • Be a resident of select communities north of Boston

Frequently Asked Questions

How do I get started as a new patient?
Call our care coordinator at (888) 281-0011. During the initial telephone call, our care coordinator will gather basic information and conduct a brief pre-screening with you over the phone. If our services are appropriate for your needs, our care coordinator will send you a new patient welcome packet with forms to be filled out and collected by the clinician at your first visit. The patient must NOT be in need of an emergency first visit.
Do I need to be homebound to become a HomeMD patient?
You do not need to be homebound to become a patient of HomeMD. Patients who are home limited and have difficulty leaving their homes for appointments are good candidates for the HomeMD primary care at home program.
Will my HomeMD clinician be my primary care provider?
Yes. When you apply to be a HomeMD patient, we ask that you sign a release stating that our clinician will take over as your primary care provider.
